Below is important information for patients who are new to Paradigm Acupuncture or are new to acupuncture treatments in general.
All new patients will need to complete a Patient History Form & Chief Complaint Form (called the MYMOP) as well as an Informed Consent prior to the first appointment. You may print the forms here, fill them out and bring them with you to your first appointment. You may also obtain the forms at the office and complete them there, just make sure to arrive at least 15 minutes prior to your appointment to allow sufficient time to fill them out.
Some tips for your appointment that will ensure a smoother and more enjoyable treatment:
1. Make sure to eat a light meal within an hour or two of your appointment. This will keep you from becoming light headed during treatment. Feel free to bring a snack (nuts, a bar, etc.) to munch on before your treatment if necessary.
2. Ideally, bring or wear loose fitting clothing, or layers that are easily adjusted during treatment. Typical areas needed for needling include the extremities (just above the knees or elbows), head and upper back. On occasion, access to the low back or abdomen may be needed. You can always change into your clothes when you arrive if you aren’t already wearing them.
3. Most importantly, make sure to arrive on time! Appointments are made within specific slots. Lateness only reduces the amount of time for your treatment. Patients who arrive more than 20 minutes late for a regular appointment, or 10 minutes late for an auricular detox appointment may be asked to reschedule.
Cancellations should be made no less than 24 hours in advance. A 50% fee must be paid for cancellations made with less than 24-hours notice, or for no-shows, before another appointment will be made.
